What You'll Do:

Provide skilled nursing care to sub-acute rehabilitation patients, including assessments, care planning, medication administration, and treatments.
Monitor patient conditions, recognize changes in status, and communicate promptly with physicians and the interdisciplinary rehab team.
Collaborate closely with PT, OT, ST, Case Management, and Social Services to support rehab goals and safe discharge planning.
Supervise and delegate nursing tasks to LPNs and CNAs in accordance with Massachusetts scope-of-practice regulations.
Accurately document assessments, treatments, and patient responses in the electronic medical record.
Ensure compliance with CMS regulations, Massachusetts DPH standards, infection control, safety protocols, and patient rights.

What You'll Need:

Active, unencumbered Massachusetts RN licensure (required.)
Minimum two (2) years RN experience preferred; sub-acute, rehab, or skilled nursing experience (required.)
PointClickCare (PCC) experience preferred
CPR certification.
Demonstrate physical ability to stand, sit, walk, bend, squat, and lift/carry up to 20 pounds unassisted or 50 pounds with assistance.
Proficient in reading, writing, and following instructions in English.
